Forms Needed Please arrive fifteen minutes before your scheduled appointment to allow yourself time to complete the necessary paperwork. Please bring the following to your appointment: 1) Requisition - this is the order your physician wrote requesting this exam for you 2) Insurance Cards - you will need current insurance cards for whomever is paying the bill (self, spouse, or parent) 3) Contact Information - please bring the name and telephone number of a friend or relative who does not live with you 4) Payment - your deductible payment is required at the time services are rendered.
